Knights Templar Maryland license plates instructions:
- Print out and complete the “Application/Certification for Organization License Plates”, (VR-124), PDF
- Complete and print out the Knights Templar Eye Foundation, Inc. “Ways to Give Financial Support” form. PDF
- Write out two checks:
- One to the “Motor Vehicle Administration” for $25.00.
- And one to the “Knights Templar Eye Foundation, Inc.” for $30.00.
- Return both completed applications and both checks to:

- Once received, the forms and checks will be sent to their corresponding destination, and you will receive your plates through the US mail from the State of Maryland.
